Hospital Discharge with our Homefirst Service
Assist Care Group is pleased to announce it has been awarded a Hospital Discharge (Homefirst) contract to the NHS and a number of block contracts to WSCC commissioning, in a large push into the public sector.
Having operated Homefirst for 7 months, Assist Care Group has developed the skills and relationships to accept referrals within 2 hours every day of the week and start packages the same day.
The new long-term contracts will provide stability to Assist Care Group and will allow the expansion of salaried carers, ensuring frontline staff enjoy consistent a reliable employment.

Homefirst – staying safe at home
Assist Care Group operates the Homefirst contract in West Sussex serving Worthing and Chichester Hospital. Since the service launched in May 2020, they have helped between 20 and 50 people per month get a swifter discharge into the safety of their own homes. Working closely with colleagues in the NHS and West Sussex Council, they have a 1.5 hours response time, 7 days a week.
The service has proved that people recuperate more quickly at home and responsive service helps prevent isolation and repeated hospital visits.
The service supports individuals with up to six weeks of free personal care package, which includes practical help, companionship, confidence building and support to ensure the home environment can also support recovery.
